Dillon Cheney: I had screwed up my drive loading a linux boot onto it for install. If clean doesn't work, just follow the process. I got errors using 'clean' also. Just continue to the next commands listed and it will work. I've used this video like 4 times and it always works, despite errors during 'clean'

hoang bach: check the size, if you know the USB size than you have a better understand of the disk. If you want to be more safe, run list disk and remember what disk are there, eject the USB you want to format, than the disk will disappear in the list disk, than would help you narrow down which disk you are looking for @_@ how that make sense and it help you out

Chiel Buseyne: You need to use 'CLEAN ALL'. If you don't put 'ALL' after 'CLEAN', only the first and the last Megabyte off the disk are deleted.
